DISCLAIMER: Medical Weight Loss at Trinity Medical Associates will bill your insurance if you provide it, however there is no guarantee that your insurance will cover all or any of the cost.  No Medical Food items or products will be released without up front payment.  All payments should be made at time of service, unless other arrangements have been made with the Director. Payment may be made by cash, personal check, or charged to your credit card. If at any time you have questions about your payment schedule, please see the Director. If your insurance does reimburse we will refund the amount the insurance pays. The financial responsibility of the costs are the patients. Meal replacements are an important part of the program and are never covered by insurance plans. Patient is responsible for losing weight every week. If the patient fails to lose weight weekly, they may not be eligible to continue the program. Patients must attend all scheduled visits; virtual and live or will risk being removed from the weight loss program, no refunds will be given for lack of participation.
